Knoxxi Games Health Declaration & Liability Waiver
1. Health Declaration
- I am physically and mentally fit to participate in the Knoxxi Games.
- I have no medical conditions or recent illnesses that could put me at risk during participation.
- I have not been advised by a healthcare professional to avoid physical activity.
- I will not participate if I feel unwell or have symptoms such as chest pain, dizziness, fainting, irregular heartbeat, or fever.
2. Assumption of Risk & Release of Liability
- I understand that participation in the Knoxxi Games involves physical activity and inherent risks, including potential injury, dehydration, or other complications.
- I voluntarily accept all risks associated with my participation.
- I release and discharge Knoxxi Health, its officers, employees, volunteers, sponsors, and partners from any liability or claims for injury, loss, or damage that may arise from my participation, to the fullest extent permitted by law.
3. Indemnification
I agree to indemnify and hold harmless Knoxxi Health and its affiliates against any claims, damages, or costs (including legal fees) resulting from my participation in the Knoxxi Games.
4. Emergency Medical Consent
I authorize the Knoxxi Health event team or emergency responders to provide or arrange first aid or medical treatment if needed, and I accept responsibility for any related costs.
5. Consent Acknowledgement
By checking the box, I confirm that I have read, understood, and agree to the above Health Declaration and Waiver. I acknowledge that this consent is legally binding as if I had signed it.
